Tip Rhinoplasty

If tip rhinoplasty had been available years ago, maybe the Wicked Witch of the West would not have been so crabby and that goody two shoes Glenda would not have hogged all the good press. Think what improvement to the nose tip would have meant to the afore mentioned witch, Pinnochio, Cyrano de Bergerac, and countless others afflicted with a large ugly nose. The tip of the nose is the smallest third of external surface of the nose, but the most complex. The smallest change in shape and angulation can have a profound effect on the appearance. It is also the only feature of the nose that is unique to every person and accounts for a large percentage of rhinoplasties.

A tip rhinoplasty can change the volume, angle and height of the nasal tip. The surgery can be simple or very complex depending on the degree of the problem. Many outcomes are possible depending on the patient's wishes and the surgeons aesthetic sense.

Listen to your surgeon. He will guide you to the best shape for your face. It can be as simple as reducing the bulbous tip cartilage or more complex, requiring the use of cartilage grafts. Most surgeons prefer to use the patient's own cartilage, which can be harvested from the septum, the ear, or even the ribs. Man made materials such as alloplastics are not recommended. The nasal tip is too mobile and exposed to tolerate artificial materials for a lifetime. Care must be taken not to reduce the tip too aggressively. It can result in a pinched look and collapse of the nasal structure. Michael Jackson is a good example of too much plastic surgery of the nose. When the nasal structure collapses, it can cause breathing difficulty and revision surgery is made much more difficult.

Nasal tip surgery can be either open or closed. In the closed procedure, the incisions are made inside the nose resulting in no external scars The open procedure is preferred by some surgeons as they feel that they have better control of the shaping process. The incision is made in the area between the nostrils and scarring is minimal. Edema is a problem with very thick skin. It could take one to two years to see final results.If swelling is excessive, it may be treated with injections of a dilute steroid solution.Complications including asymmetry, bossa formation, and polly beak deformity can occur with excessive deprojection, inadequate resection,or persistent edema. Massive collapse can occur with excessive tip narrowing, eg. Michael Jackson, decreasing the nasal airway.

A good surgeon will make recommendations to prevent complications. Sometimes a "little tweak" is all that is needed to make a big improvement in your appearance.
